At your first visit with us, you will be asked to sign the treatment consent form stating that you have provided the truth about your health history, read the information, understand it, and agree to comply with professional massage therapy policies and procedures. If the client fails to give informed consent, the session will not be able to begin unless the client agrees to sign. Clients whom we have not seen for at least 6 months may also be asked to fill out this form again.

            It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently being taken and to provide written permission from the physician, chiropractor, physical therapist, etc., if needed.
